What happens next?  

Locala, who currently run Calder Community Practice, will still provide GP services until Friday 28 June 2024.  

In June patients of Calder Community Practice who live in Calderdale will be automatically registered with the nearest GP practice to their home address that is accepting new patients. You do not need to do anything.  

We will write to patients confirm which practice they have been registered with.

2,857 patients are registered with Calder Community Practice.

 

My care and medication

What if I am receiving ongoing, immediate care? 

Calder Community Practice will provide details of all patients receiving ongoing care, or those in need of immediate care to their new GP practice to ensure those whose health requires close monitoring receive the care they need.

 

Will I be able to contact my new GP practice like I do now? How can I make an appointment?

All GP practices in Calderdale offer face to face, online and telephone access to book appointments. 

You'll find more details about how to do this on the GP practice's website when we have confirmed which practice you're registered with.

If you're unable to access the website, give your new practice a call, or pop in to meet the team.

 

What happens to my repeat prescription(s)?  

Your new GP provider will have access to all your medical records. You should be able to get all your repeat prescriptions from them just as you do now. If you have a nominated pharmacy, this will remain the same too.  

As part of moving to your new GP practice you may be invited for a medication review if one is due. 

For example: if you have a medication on repeat which is reviewed every six months. If the six-monthly review falls when you are being re-registered, then you would still need a medication review.  However, if you are only three months in, then your medication would continue as normal until you reach the usual six months review period. 

 

How will my medical record be transferred to my new GP practice? 

When you change GP practices your medical record will be sent through a secure electronic route from Calder Community Practice directly to your new GP surgery.  

Once your record has been received by your new surgery it may take up to one week for your record to be updated to reflect your new GP surgery registration.   

Some patients may also have a paper medical record. This will be placed in a trackable envelope, collected from Calder Community Practice and delivered directly and securely by a national approved NHS courier to your new practice.

 

Will I have to travel further for an appointment? 

The ICB will allocate you to the GP practice nearest to your home postcode.

 

My new GP practice will be Todmorden Group Practice. I’ve heard you can't get an appointment there unless you ring or attend the practice at 8am.  

Todmorden Group Practice has made considerable improvements since summer 2023 in line with the General Practice access model recommended by NHS England. 

This includes the recruitment of more staff, including 2 new GP partners, additional GPs and other practice staff. 

Appointments can be booked online through the practice website, on the phone, or through the NHS App. Same day appointments are available throughout the day, up until 5pm.  

 

The decision making process

Why can’t the current provider continue delivering GP services from the practice? 

The contract with the current provider for Calder Community Practice is a time-limited contract which has been extended twice.  

NHS West Yorkshire ICB is unable to extend the contract with the current provider beyond 28 June 2024.

 

Does Calder Community Practice have to close?  

The ICB ran a thorough procurement process, and the most suitable provider was selected and offered a contract. 

West Yorkshire ICB has not been able to find another provider to deliver GP services from Calder Community Practice and the practice will close on 28 June 2024. Patients who live in Calderdale will be re-registered with a new surgery.

 

How have you worked with patients and the local community to make this decision?  

In 2019 we asked patients how they felt about the Calder Community Practice and GP services in Todmorden. We also looked at the latest National GP Patient Survey results for the practice.  

We used this information to help inform our procurement process. 

As we have not been able to appoint another provider, the practice will unfortunately close as previously advised.  

Involving people would not be able to influence or change the decision that we have had to make. We have listened to the feedback that we have received since starting this process and we understand the disappointment and frustration that will be felt in the community because of the practice closing.

Can Hebden Bridge Group Practice withdraw from the process? 

Yes, Hebden Bridge Group Practice can withdraw from the process under NHS procurement rules.

 

Is it possible that a new procurement process will start again in the future?   

There are no plans to run another procurement process. Closing the practice and registering patients with a different GP practice is the only option.  

 

How is the ICB communicating this information?   

Patients: All households registered with Calder Community Practice have been sent a letter informing them what is happening. The ICB will contact these patients again in June to confirm which GP practice they have been registered with from Monday 1 July 2024. 

Local councillors and MPs: updates have been shared with local councillors, MPs and all relevant committees.  

Practice staff with Calder Community Practice: the ICB has worked closely with Locala throughout this process, and practice staff have been informed. 

Local health providers, including other GP practices in the area: updates have been shared with all GP practices in Calderdale and pharmacists in a three-mile radius of Calder Community Practice. 

Health partners: partners in Calderdale’s health and care sector, including Healthwatch Calderdale. Local advocacy organisations have also been informed.

 

Is this about money? Could more money have been offered to keep the practice open?   

The contract offered was based on the standard financial rate for running a GP practice. The financial part of the contract is made up of funding for each patient. This is called the ‘Global Sum.’ This makes up the majority of the funding and is a guaranteed income for the provider.   

Calder Community Practice has a lower number of registered patients compared to other practices in Calderdale.   

We are responsible for managing the NHS budget on behalf of all Calderdale residents and are committed to fair funding. We are unable to increase the value of the contract.

 

NHS West Yorkshire Integrated Care Board and Calderdale Cares Partnership

What is NHS West Yorkshire Integrated Care Board?    

NHS West Yorkshire Integrated Care Board (ICB) oversees the planning and buying of NHS services for local people. 

We are responsible for ensuring that people in Calderdale have access to GP services. 

NHS West Yorkshire Integrated Care Board become a statutory organisation on 1 July 2022 and saw the transfer of all the clinical commission groups in West Yorkshire. This includes NHS Calderdale Clinical Commissioning Group (CCG).    

 

What is the relationship between the NHS West Yorkshire ICB and Calderdale Cares Partnership?    

Calderdale Cares Partnership is a number of local organisations working together to improve the health and wellbeing of local people.  

The Calderdale Cares Partnership Board is one of five place-based committees of the NHS West Yorkshire Integrated Care Board. Place-based committees are responsible for making decisions about how NHS resources are used in their local area.

 

Why have you waited until now to tell patients the practice is closing?   

When Hebden Bridge Group Practice withdrew from the process, we explored all other available options within the procurement process to secure another GP provider. This process ended on 12 March 2024. We have communicated with patients as soon as possible.

 

Privacy Information

As the local commissioner of Primary Care Services in Calderdale, NHS West Yorkshire Integrated Care Board (Calderdale Cares Partnership), have an obligation to ensure you have access to GP services. As part of this role NHS England have provided us with access to the following details of patients registered with Calder Community Practice

  • name
  • address
  • postcode
  • date of birth (to identify children under 18 so letter communications can be sent to the parent of the patient at the same address)
  • NHS number

for the purposes of corresponding with registered patients about the closure of Calder Community Practice.  

The lawful basis we are relying on to use registered patients' personal data is UK General Data Protection Regulation Article 6 – (e) Processing is necessary for the performance of a task carried out in the public interest or in the exercise of official authority vested in the controller.

We will keep personal data securely on our files for 10 years following the closure of the Practice.

My new GP practice

Can I choose which practice I’m registered with?   

If you would like to register with a different GP practice yourself, you can use the Find a GP service at www.nhs.uk to look for a surgery near you, or outside your local area, that is accepting new patients.  

You will need to fill in a registration form that you can get from your chosen GP surgery, the GP surgery’s website, the GP surgery’s profile page on the NHS website, or from the NHS App.

Are other GP practices in Calderdale able to accept patients from Calder Community Practice? 

Yes, all Calderdale GP practices have ‘open lists’ and are accepting new patients.  

The ICB is working with the practices closest to your home address to ensure that they are ready to welcome you as a new patient. 

If you/a patient wants to register with a practice outside of your area, then the practice may accept you but may not be able to provide home visits or other services.

 

Will I have to travel further for an appointment? 

The ICB will allocate you to the GP practice nearest to your home postcode.

 

What if I don’t want to be registered with the practice I’m allocated?

What if I don’t live in Calderdale? 

NHS West Yorkshire ICB are not able to register people living outside of Calderdale with a new GP practice.

I live in Littleborough, will I be registered with a Calderdale practice?
