Purpose
An evaluation of the proactive care programme is taking place in Leeds. The evaluation aims to understand the perspective of patients and carers about proactive care in the Leeds area. Proactive care is a multi-agency approach to health and wellbeing that identifies and supports the needs of people and their carers before their health deteriorates significantly. It focuses on early identification and intervention to keep individuals well, independent and connected to their communities for as long as possible. Support is based around what matters to the person and aims to give people choice and control over the way their care is planned and delivered.
The evaluation of the proactive care programme will include interviews of patients, their carers and family members to ask about the quality of care they receive. The interviews will be carried out by Leeds Older People’s Forum (LOPF) or that persons local neighbourhood network. Local neighbourhood networks are established parts of the voluntary and community sector in Leeds, and they have been commissioned by Leeds Older People’s Forum to help support the evaluation work.

Person identifiable information including names, dates of birth, email address and phone numbers.
Who we will share the information with (recipients)
Contact details (person identifiable information) will be shared with a small number of Third sector providers in Leeds who will interview the patient and enter their anonymised responses into a secure electronic form owned and managed by West Yorkshire ICB.
Do we use any processors
Yes, third sector provider - Leeds Older Peoples Forum.
A small number of neighbourhood networks will support the evaluation activity as sub data processors to Leeds Older Peoples Forum: Aireborough Voluntary Services to the Elderly (AVSED), Burmantofts Community Friends, Cross Gates Good Neighbours, and Older People’s Action in the Locality (OPAL).
How we collect (the source) and use the information
Contact details (personal data) will be collected directly from the patient by a clinician in the patients registered GP practice.
How long we will keep the information
The third sector providers, including LOPF, will hold the contact details until the evaluation report is signed off by the Leeds Place of the West Yorkshire Integrated Care Board (ICB). The ICB will hold this contact information in line with their record retention and destruction schedule.
Your rights
Under the UK General Data Protection Regulation all individuals have certain rights in relation to the information which the ICB holds about them. Not all rights apply equally to all our processing activities as certain rights are not available depending on the lawful basis for the processing.
